Three claims, and what the records behind them actually say

These claims and participants are invented. The assessment is not — this page runs the same engine that runs on a provider's own imported data, at the moment you load it.

Claim value reviewed

$2,769

Across 3 claims.

Evidence coverage

17.6%

The share of assessed claim value that has a record linked to it and no unresolved evidence issue.

Claim value requiring attention

$2,281

Not money you will lose, not a clawback estimate, and not a prediction of any NDIA decision. It is the amount worth someone looking at.

The three cases every provider recognises

Why this one is here: The ordinary case. The note describes the support that was claimed, for about as long as was claimed, and connects it to a plan goal.

A record is linked to this claim and nothing in it contradicts what was claimed. This is ShiftNote’s reading of the documents you gave it. It is not a decision by the NDIA and does not predict one.

What to do: Nothing to do. Keep the record retrievable.

What was checked

  • Evidences the support item claimedPassed

The note describes personal care / daily living (shower, dressing, personal care).

  • Duration matches the claimPassed

The note evidences 120 minutes against 120 claimed.

  • Linked to a plan goalPassed

Linked to: Build independence in morning personal care routine

  • Objective and specificPassed

No subjective or judgemental language detected.

  • Required elements presentPassed

    All checked elements are present.

  • Nothing contradicting the claimPassed

Nothing in the note contradicts the claim.

Assessed by ShiftNote engine v1.0.0. Dimensions the data could not answer are excluded from the result rather than scored zero. This is a heuristic reading of the records, not a determination about the claim. How the assessment works.

Read the linked record

PURPOSE OF SHIFT: Scheduled 2 hours of morning personal care support at P-0148’s home, as set out in their service agreement. SUPPORT PROVIDED: Arrived 9:00am and finished 11:00am, 2 hours in total. Supported P-0148 through showering and dressing with standby assistance and verbal prompting only. Prepared breakfast together; P-0148 chose and buttered their own toast. PARTICIPANT RESPONSE & OBSERVATIONS: P-0148 completed the sequence with two verbal prompts. Four were needed at the same task last week. P-0148 talked about the weekend throughout and asked to keep the same order of tasks tomorrow. PROGRESS TOWARD NDIS GOALS: This shift worked on the goal of building independence in the morning personal care routine. The reduction from four prompts to two at the dressing step is progress toward completing the routine with standby support only. HEALTH, WELLBEING & MEDICATION: No medication administered this shift. No health concerns observed. P-0148 ate a full breakfast. INCIDENTS, RESTRICTIVE PRACTICES & FOLLOW-UP: No incidents and no restrictive practices used. Next shift to continue reducing prompts at the dressing step and record how many are needed.

Why this one is here: Six hours claimed, a note describing about one. This is the case that costs providers money, and no claim validation tool sees it, because the claim itself is perfectly well formed.

The linked record and the claim disagree — different dates, a duration the note does not support, or a support the note does not describe.

What to do: Reconcile the two before anyone else does. One of them is wrong and only you can say which.

What was checked

  • Evidences the support item claimedPartial

The note describes community participation (shops).

  • Duration matches the claimNot evidenced

The note evidences 60 minutes but 360 were claimed.

  • Linked to a plan goalNot evidenced

The note does not connect the support to any goal in the plan.

  • Objective and specificPartial

    1 subjective term detected.

  • Required elements presentNot evidenced

Missing: Goal linked, Outcome noted, Engagement described, Follow-up captured, Concerns addressed.

  • Nothing contradicting the claimPassed

Nothing in the note contradicts the claim.

Issues to resolve

  • Resolve first

The note evidences about 1 hours but 6 hours were claimed.

Correct the note or the claim so the hours match. A duration mismatch is one of the first things a payment assurance review looks for.

  • Resolve first

The note is missing goal linked, outcome noted, engagement described, follow-up captured, concerns addressed.

Complete the missing sections before this note is used to substantiate a claim.

  • Worth fixing

The note only weakly evidences community participation.

Describe the community participation support you actually delivered — what you did, and what the participant did.

  • Worth fixing

The note does not link the support to a goal in the participant's plan.

Add a sentence connecting what you did to one of the plan goals (e.g. "Increase participation in community activities").

  • Minor

The note uses subjective language (happy).

Replace opinion words with what you observed. Subjective notes are harder to defend under review.

Took P-0203 to the shops for about 1 hour. Picked up a few things they needed and had a coffee. P-0203 was quiet but happy enough. Dropped home after.

Why this one is here: Delivered and billed, but nothing was ever written up — or the note is in a system nobody reconciled against the claim. It is reported as missing evidence, not scored as weak, because there is nothing to read.

Something the claim depends on is not there at all — most often no note is linked to it.

What to do: Find the record, or link the one that exists. If no record was ever written, that is what you need to know first.

What was checked

There is nothing to assess.

No record is linked to this claim, so ShiftNote does not run the checks and does not produce a score. Reporting a zero here would read as “we assessed the note and it failed” about a note that does not exist.

Where the deadline on each claim comes from

The days-remaining figure on each claim above is arithmetic on the delivery date and the claiming window that applies to it. Both windows are in play right now, so the date a support was delivered decides which one it gets.

What the rule says

From 1 December 2026 the period for claiming a support delivered under a participant’s plan drops from two years to 90 days.

Applies to: Providers claiming against a participant’s NDIS plan for supports delivered on or after 1 December 2026.

Applies from: 1 December 2026

Where this is not the whole answer

Supports delivered before 1 December 2026 keep the window that applied when they were delivered. Confirm how the change applies to your own service bookings with the NDIA.

These example shifts were delivered before 1 December 2026, so they keep the two-year window that applied when they were delivered — which is why the deadlines above read in hundreds of days rather than tens. The same shifts delivered after that date would have 90 days. Both rules are live at once, and that is the part providers importing several months of history get wrong.

ShiftNote provides software and operational information, not legal advice. Providers remain responsible for understanding the requirements that apply to their own circumstances.

Run this on your own claims

The free exposure audit runs in your browser — your claims file never reaches our server. It shows claim-window exposure from your dates. Evidence exposure needs your notes as well, which is what the Evidence Exposure Report is for.
